Hourly Job Coaches - Part Time

ASSIGNMENT:        Various Locations 


  • High School Diploma or Equivalency
  • Demonstrate commitment to valuing diversity and contributing to an inclusive working and learning environment


  • Non-traditional work hours based on consumer need, 2 to 3 hours per week/per consumer (typically 5-20 hours/week)
  • Provide job coaching skills training and transportation training to consumers in various workplace locations throughout Suffolk County (multiple positions, multiple locations – days, evenings and weekends)
  • Provide work related social skills training
  • Provide employer training as needed and perform job development
  • Participate in care coordination meetings
  • Completion of time sensitive documentation as required by funding services
  • Must have strong organizational and time management skills
  • Proficiency in writing skills and Microsoft Office

START DATE:       As soon as possible
