In-District Professional Development Request Form

2026-2027 In-District Professional Development Request Form 

This form is for districts requesting to work with a consultant or vendor through Eastern Suffolk BOCES.

Before completing the form, the district should confirm the professional development details and proposed date(s) directly with the consultant or vendor.

Districts that need assistance identifying a consultant may email pdreferral@esboces.org, with the following information:

• Professional development goals
• Intended audience
• Anticipated start date
• Preferred consultant, if applicable

If the district has a preferred consultant, our team will help facilitate the connection. If no consultant has been identified, we will provide several options for the district to consider.

Once submitted, the information on this form will be processed and applied to the district's PD commitment funds.  If there are insufficient funds, a Contract Modification Request (CMR) will be generated and emailed to the district contact person for the Superintendent's signature.

All required paperwork must be fully executed and received by pdteam@esboces.org before professional development services may begin.
