** NEW Online ** 2024-2025 In-District Professional Development Request Form (Please use online submission form)

Please use the online submission form for all future planning requests. If you have any issues, please email pdonlinesupport@esboces.org for assistance.

This form is used when districts would like to work with a consultant/vendor through Eastern Suffolk BOCES. 

• The district must first schedule the professional development with the consultant/vendor and then complete this form.
• We will use this form to generate a Contract Modification for the district Superintendent to sign.
• This paperwork must be in place before the work begins in the district.
• Email the form to us as an attachment to pdteam@esboces.org.
