Professional Services Contract (PSC) Directions
Professional Services Contracts (PSCs) are emailed directly to consultants for review and completion.
Michelle Donovan mdonovan@esboces.org will send your PSC and serves as your primary point of contact for any questions related to the contract.

📅 Contract Details
• Contract Term: MONTH DAY, 20XX to June 30, 20XX (Check to ensure that the start date will cover any anticipated in-district work)
• Not to Exceed Amount: $XXXXX
All work performed for Eastern Suffolk BOCES or our component districts must occur within this contract term.
We ask that you carefully track your consulting hours. We recommend that you keep your own spreadsheet for both regional workshops and any in-district professional development.
If you are approaching the halfway point of the total not to exceed amount and anticipate the need for additional work, please email pdteam@esboces.org to discuss a possible amendment.
✍️ Required Documents
Please review and complete the following:
• PSC: Initial in the bottom left corner of each page and sign and date in the Contractor section at the end of the agreement.
(If your contract includes a middle name or middle initial, ensure it is used consistently for all initials and signatures throughout the document.)
• NYS Retirement System Form:
Sign and date appropriate sections
• Disclosure Form: Ensure accuracy of officers, sign and date
• W9: Please provide an updated W-9 if there have been any changes to your address, name, business name, or tax filing status.
• Letter of Authority (if applicable):
Required if the PSC signer is not a COO, CEO, CFO, President, or VP
