Community School Resources Program
Supporting Whole-Child Wellness Through Strategic Community Partnerships
Eastern Suffolk BOCES offers a comprehensive Community School Resources CoSer (CoSer 535) designed to expand district capacity in addressing student mental health, wellness, and overall well-being.
Through this CoSer, districts partner with leading healthcare and community organizations to provide coordinated, multi-tiered supports for students, families, and staff, including both in-district consultation and access to specialized clinical services when appropriate.
How the Community School Resources CoSer Works
Participation in CoSer 535 provides districts with:
- A coordinated partnership model connecting schools with trusted healthcare providers
- Access to multiple service tiers, including consultative, preventative, and responsive supports
- BOCES coordination and oversight, including contracts, scheduling, and program management
- Ongoing collaboration structures, such as leadership meetings, case consultation, and data-informed planning
- Priority access to services and resources through established partnerships
Districts may participate in one or more partnership programs based on their needs.
What This CoSer Provides
Student & Family Support Services
- Crisis response, risk assessment, and postvention support
- Mental health evaluations and referral coordination
- Short-term intervention and care navigation
- Family engagement and support resources
Consultation & Systems Support
- Ongoing collaboration with district leadership and support teams
- Case consultation and multidisciplinary problem-solving
- Guidance on protocols, policies, and best practices
- Integration of mental health supports within MTSS frameworks
Professional Learning
- Staff training led by clinical and educational experts
- Workshops, webinars, and ongoing learning series
- Access to specialized topics and emerging best practices
Community & Care Coordination
- Assistance connecting families to services and insurance resources
- Streamlined access to partner organizations
- Coordination across school, home, and provider systems
