| 1. | Intake information forms are the first step in the registration process to the Secondary Career and Technical Education Technical Centers and to the Special Career Education Centers. The Intake Information form must be completed, signed appropriately and received before a student will be enrolled into an Eastern Suffolk BOCES program. Students should not be sent to an Eastern Suffolk BOCES program until the sending district has received confirmation of admission. All intake information forms should be printed neatly in ink or typewritten. (When filling out the form in ink, make sure all copies are legible.)

| 2. | A new intake information form must be filed for each student. The form must be filed each year the student is enrolled.
The Intake Form must be accompanied by the following supportive documentation: high school transcript, IEP (current), Level II Assessment, current Psychological (strongly suggested). Students are no longer maintained on the active roster until formally dropped by the student's district. A new intake form must be submitted each year for every student.
Students transferring between Secondary Career and Technical programs and Special Career Education programs must submit a new intake information form as per procedures above.

| 3. | Students attending parochial or private schools must have home school district approval. A student attaining post-graduate status that the district agrees to continue in a Secondary Career Education enrollment must have an Intake Information form submitted which ensures district approval. This approval will be signified by the signature of an authorizing school district official appearing on the Eastern Suffolk BOCES form on the line entitled "Signature of Tuition School District."

| 4. | A student classified by their district CSE must have supportive information concerning the disability submitted along with the Career and Technical Education Intake Information form and the appropriate box on the form must be checked. Supportive data is defined as a current I.E.P., psychological or summary report, medical report, school grade reports or transcript and a Level II vocational assessment report. Please be sure to check the space on the Intake form to indicate the program in which a student is to be enrolled, e.g., Secondary Career and Technical Education or Special Career Education.

| 5. | When recording the grade of the student, record the grade level in which the student is enrolled at the time or date that the application is completed.

| 8. | Adult residents of the supervisory district who are over 21 or have received a high school diploma may enroll in secondary programs, but only if seating is available. Priority is given to secondary students and former Eastern Suffolk BOCES students who have completed the first year of a two-year program. Adults will be billed three times per year (September, December and March) on the first school day of the month. This policy does not include adults enrolled in Eastern Suffolk BOCES Adult Career and Continuing Education programs.
Intake Information forms for adults are available in the Admissions Office and completed forms must be returned to the Adult Career Education Office. For additional information contact the Edward J.
